If you received a survey form last week (Thursday or Friday) it is extremely important that you fill it out on both sides and turn it into the box at the front of the Balboa Park Activity Center. Not only is it important to turn it in so that the city can see the results, but also because you cannot get another one! They have passed out the maximum amount of forms allocated to the building, so that’s it!
